    OpenEMR 8.2.0 Stored XSS via import_template.php Template Management
    Summary
    OpenEMR through 8.2.0 contains a stored cross-site scripting vulnerability in the patient portal template system that allows authenticated administrators to inject arbitrary HTML and JavaScript by storing malicious payloads through the template save mode, which only filters literal PHP open tags. Attackers can exploit the lack of output encoding at the template retrieval endpoint combined with missing HttpOnly cookie attributes to exfiltrate session tokens via document.cookie access, enabling full session hijacking of any admin, clinician, or portal patient who views a poisoned template.

    OpenEMR 8.2.0 OAuth2 Password Grant Authentication Bypass via SMART Configuration
    Summary
    OpenEMR through 8.2.0 contains an authentication bypass vulnerability that allows attackers with valid credentials to circumvent multi-factor authentication by exploiting the exposed OAuth2 password grant flow through an unauthenticated client registration endpoint. Attackers can register an OAuth2 client via the unauthenticated registration endpoint and use the password grant to exchange credentials for an API access token, bypassing the normal web interface authentication and any enforced multi-factor authentication controls.

    Summary
    OpenEMR through 8.2.0 contains an improper authentication vulnerability in the OAuth2 dynamic client registration endpoint that allows unauthenticated attackers to register a malicious client with system-level FHIR scopes by supplying a self-generated RSA keypair via the jwks field. Once an administrator approves the registered client, attackers can use the client_credentials grant with a self-signed JWT assertion to obtain access tokens granting read access to all FHIR resources across all patients in the system.

    OpenEMR 8.2.0 Remote Code Execution via CategoryTree eval() Injection
    Summary
    OpenEMR through 8.2.0 contains a remote code execution vulnerability in the document category tree component (library/classes/Tree.class.php) that allows authenticated administrators to execute arbitrary operating system commands by injecting PHP payloads into the categories database table. Attackers can chain arbitrary SQL execution to alter the id column type to VARCHAR and insert a malicious PHP payload, which is then executed via an unsanitized eval() call whenever any page instantiates CategoryTree, including unauthenticated and low-privilege pages, resulting in command execution as the web server user.

    OpenEMR Authenticated SQL Injection via backup.php Import Feature
    Summary
    OpenEMR through 8.2.0 contains an authenticated SQL injection vulnerability in the backup configuration import feature that allows administrators with admin or super ACL privileges to execute arbitrary DDL and DML statements against the application database by uploading a crafted SQL file at the form_step=202 parameter in backup.php. Attackers can exploit the unfiltered shell_exec invocation of the mysql command-line client to extract credential hashes, modify access control tables, inject backdoor accounts, create persistent triggers or stored procedures, and write arbitrary files to the filesystem where MySQL FILE privileges and permissive secure_file_priv settings are configured.

    OpenEMR: Stored XSS in prescription CSS/HTML print view via patient demographics
    Summary
    OpenEMR is a free and open source electronic health records and medical practice management application. Prior to version 8.0.0.1, a stored cross-site scripting vulnerability in the prescription CSS/HTML multi-print feature allows a patient portal user to execute arbitrary JavaScript in a clinician's browser session. Patient demographic fields (name, address) are rendered without output encoding in multiprintcss_header(), and portal patients can write attacker-controlled HTML directly into patient_data by calling the PUT api/patient/:num endpoint, which bypasses the intended audit review workflow. Because the XSS fires in the clinician's authenticated session on the main OpenEMR interface, the attacker can access CSRF tokens, session data, and perform actions as the clinician — crossing the patient-to-clinician trust boundary. This issue has been patched in version 8.0.0.1.

Multiple web UI callers pass user-controlled note IDs directly to these functions. This is the same class of vulnerability as CVE-2026-25745 (REST API IDOR), but affects the web UI code paths. Version 8.0.0.3 patches the issue.

    OpenEMR Missing Authorization in Procedure Order AJAX Deletion Handler

    OpenEMR is a free and open source electronic health records and medical practice management application. Prior to version 8.0.0.3, missing authorization in the AJAX deletion endpoint `interface/forms/procedure_order/handle_deletions.php` allows any authenticated user, regardless of role, to irreversibly delete procedure orders, answers, and specimens belonging to any patient in the system.     Title
    OpenEMR has SQL Injection in PostCalendar Category Delete
    Summary
    OpenEMR is a free and open source electronic health records and medical practice management application. Prior to version 8.0.0.3, the PostCalendar module contains a blind SQL injection vulnerability in the `categoriesUpdate` administrative function. The `dels` POST parameter is read via `pnVarCleanFromInput()`, which only strips HTML tags and performs no SQL escaping. The value is then interpolated directly into a raw SQL `DELETE` statement that is executed unsanitized via Doctrine DBAL's `executeStatement()`. Version 8.0.0.3 patches the issue.

    OpenEMR through 8.2.0 contains a stored cross-site scripting vulnerability in the patient portal template system that allows authenticated administrators to inject arbitrary HTML and JavaScript by storing malicious payloads through the template save mode, which only filters literal PHP open tags. Attackers can exploit the lack of output encoding at the template retrieval endpoint combined with missing HttpOnly cookie attributes to exfiltrate session tokens via document.cookie access, enabling full session hijacking of any admin, clinician, or portal patient who views a poisoned template.

Attackers can register an OAuth2 client via the unauthenticated registration endpoint and use the password grant to exchange credentials for an API access token, bypassing the normal web interface authentication and any enforced multi-factor authentication controls.

    OpenEMR 8.2.0 Remote Code Execution via CategoryTree eval() Injection
    Summary
    OpenEMR through 8.2.0 contains a remote code execution vulnerability in the document category tree component (library/classes/Tree.class.php) that allows authenticated administrators to execute arbitrary operating system commands by injecting PHP payloads into the categories database table. Attackers can chain arbitrary SQL execution to alter the id column type to VARCHAR and insert a malicious PHP payload, which is then executed via an unsanitized eval() call whenever any page instantiates CategoryTree, including unauthenticated and low-privilege pages, resulting in command execution as the web server user.

    OpenEMR Authenticated SQL Injection via backup.php Import Feature
    Summary
    OpenEMR through 8.2.0 contains an authenticated SQL injection vulnerability in the backup configuration import feature that allows administrators with admin or super ACL privileges to execute arbitrary DDL and DML statements against the application database by uploading a crafted SQL file at the form_step=202 parameter in backup.php. Attackers can exploit the unfiltered shell_exec invocation of the mysql command-line client to extract credential hashes, modify access control tables, inject backdoor accounts, create persistent triggers or stored procedures, and write arbitrary files to the filesystem where MySQL FILE privileges and permissive secure_file_priv settings are configured.

    OpenEMR: Stored XSS in prescription CSS/HTML print view via patient demographics
    Summary
    OpenEMR is a free and open source electronic health records and medical practice management application. Prior to version 8.0.0.1, a stored cross-site scripting vulnerability in the prescription CSS/HTML multi-print feature allows a patient portal user to execute arbitrary JavaScript in a clinician's browser session. Patient demographic fields (name, address) are rendered without output encoding in multiprintcss_header(), and portal patients can write attacker-controlled HTML directly into patient_data by calling the PUT api/patient/:num endpoint, which bypasses the intended audit review workflow. Because the XSS fires in the clinician's authenticated session on the main OpenEMR interface, the attacker can access CSRF tokens, session data, and perform actions as the clinician — crossing the patient-to-clinician trust boundary. This issue has been patched in version 8.0.0.1.

    OpenEMR has IDOR in Patient Notes Web UI allows unauthorized note access/modification
    Summary
    OpenEMR is a free and open source electronic health records and medical practice management application. Prior to version 8.0.0.3, the legacy patient notes functions in `library/pnotes.inc.php` perform updates and deletes using `WHERE id = ?` without verifying that the note belongs to a patient the user is authorized to access. Multiple web UI callers pass user-controlled note IDs directly to these functions. This is the same class of vulnerability as CVE-2026-25745 (REST API IDOR), but affects the web UI code paths. Version 8.0.0.3 patches the issue.

    Title
    Reflected XSS via Unescaped contextName Parameter in Custom Template Editor
    Summary
    OpenEMR is a free and open source electronic health records and medical practice management application. Starting in version 7.0.2.1 and prior to version 8.0.0.3, a reflected cross-site scripting (XSS) vulnerability in the custom template editor allows an attacker to execute arbitrary JavaScript in an authenticated staff member's browser session by sending them a crafted URL. The attacker does not need an OpenEMR account. Version 8.0.0.3 patches the issue.
