The product reuses or references memory after it has been freed. At some point afterward, the memory may be allocated again and saved in another pointer, while the original pointer references a location somewhere within the new allocation. Any operations using the original pointer are no longer valid because the memory "belongs" to the code that operates on the new pointer.

cachefiles: flush all requests after setting CACHEFILES_DEAD
In ondemand mode, when the daemon is processing an open request, if the kernel flags the cache as CACHEFILES_DEAD, the cachefiles_daemon_write() will always return -EIO, so the daemon can't pass the copen to the kernel. Then the kernel process that is waiting for the copen triggers a hung_task.
Since the DEAD state is irreversible, it can only be exited by closing /dev/cachefiles. Therefore, after calling cachefiles_io_error() to mark the cache as CACHEFILES_DEAD, if in ondemand mode, flush all requests to avoid the above hungtask. We may still be able to read some of the cached data before closing the fd of /dev/cachefiles.
Note that this relies on the patch that adds reference counting to the req, otherwise it may UAF.

After setting FSCACHE_CACHE_IS_WITHDRAWN, wait for all the cookie lookups to complete first, and then wait for fscache_cache->object_count == 0 to avoid the cookie exiting after the volume has been freed and triggering the above issue. Therefore call fscache_withdraw_volume() before calling cachefiles_withdraw_objects().
This way, after setting FSCACHE_CACHE_IS_WITHDRAWN, only the following two cases will occur: 1) fscache_begin_lookup fails in fscache_begin_volume_access(). 2) fscache_withdraw_volume() will ensure that fscache_count_object() has been executed before calling fscache_wait_for_objects().

qlcnic: prevent ->dcb use-after-free on qlcnic_dcb_enable() failure
adapter->dcb would get silently freed inside qlcnic_dcb_enable() in case qlcnic_dcb_attach() would return an error, which always happens under OOM conditions. This would lead to use-after-free because both of the existing callers invoke qlcnic_dcb_get_info() on the obtained pointer, which is potentially freed at that point.
Propagate errors from qlcnic_dcb_enable(), and instead free the dcb pointer at callsite using qlcnic_dcb_free(). This also removes the now unused qlcnic_clear_dcb_ops() helper, which was a simple wrapper around kfree() also causing memory leaks for partially initialized dcb.
Found by Linux Verification Center (linuxtesting.org) with the SVACE static analysis tool.
